Thank you for participating in our scholarship contest and congratulations on your winning submission. To collect your award you must complete a W-9 form and submit it along with an image of your student ID and the rest of the information in the form on this page. You can download a blank W-9 form here.  Be sure to fill it out completely and sign it before submitting.

If you would like to receive your payment faster, please provide your bank information at the bottom of this form and we will direct deposit your award.
